Investigation of the Optimum Baseline Blood Pressure for Spinal Anesthesia to Guide Vasopressor Management for Elective Cesarean Delivery: A Case-Control Design

چکیده
Background: Current guidelines recommend prophylactic vasopressor administration during spinal anesthesia for cesarean delivery to maintain intraoperative blood pressure above 90% of the baseline value. We sought determine optimum mean arterial (MAP) reading guide management hypotension. Methods: performed a secondary analysis data collected from normotensive patients presenting elective in tertiary care institution October 2018 August 2020. compared magnitude hypotension who reported nausea versus those did not, using case-control design. Baseline MAPs at last office visit, morning surgery, or operating room (pre-spinal) were determined. calculated duration and degree area under curve (AUC) when MAP respective patient was below each baseline. Results: The experienced (n=45) had longer more profound periods than not develop (n=240). A comparison AUC visit on surgery showed statistically significant between-group difference, P=0.02, P=0.005, respectively, no difference used. Conclusions: Patients highest preoperative similar with without pre-spinal Therefore, maintaining higher individual as should reduce maternal nausea.

To the Editor: We read with interest the article by Hillerman et al.1 concerning laryngeal nerve identification during thyroid surgery using the laryngeal mask airway (LMA). Although postoperative voice changes are usually attributed to recurrent laryngeal nerve injury, the external branch of the superior laryngeal nerve (EBSLN) is also at risk since it runs close to the superior thyroid artery...
متن کاملinvestigating the feasibility of a proposed model for geometric design of deployable arch structures
deployable scissor type structures are composed of the so-called scissor-like elements (sles), which are connected to each other at an intermediate point through a pivotal connection and allow them to be folded into a compact bundle for storage or transport. several sles are connected to each other in order to form units with regular polygonal plan views. the sides and radii of the polygons are...
Preload or coload for spinal anesthesia for elective Cesarean delivery: a meta-analysis.
BACKGROUND Hypotension following spinal anesthesia for Cesarean delivery is common. Fluid loading is recommended to prevent hypotension, but preload is often ineffective. In non-pregnant patients, coloading has been shown to better maintain cardiac output after spinal anesthesia. The purpose of this meta-analysis was to determine whether the timing of the fluid infusion, before (preload) or dur...
متن کاملPulmonary effects of bupivacaine and ropivacaine in parturients undergoing spinal anesthesia for elective cesarean delivery.
UNLABELLED To study the change of maternal pulmonary function when ropivacaine and bupivacaine were used in spinal anesthesia for cesarean section, 40 ASA physical status I and II parturient scheduled to undergo cesarean section were randomly divided into bupivacaine and ropivacaine groups. Bupivacaine 9 mg and ropivacaine 14 mg were intrathecal injected respectively. FVC, FEV1 and PEFR were me...

conclusions addition of meperidine or fentanyl to lidocaine and epinephrine solution increases the duration of postoperative analgesia in cesarean section. meperidine is a recommended adjuvant according to longer duration of analgesia and lower complications. results the mean duration of analgesia with meperidine, fentanyl or placebo were 9.46 ± 0.6, 6.27 ± 0.45, 2.06 ± 0.13 hours, respectively...
